About us
We provide specialist palliative care for patients with complex needs who are at the end of their lives, based on need, not diagnosis or prognosis. Complex healthcare needs are those which are difficult to manage, particularly when many problems are occurring at once. These can include physical symptoms, psychological symptoms, family distress, social issues or spiritual challenges.
Meadow House Hospice is part of London North West Healthcare (LNWH) Charity. We ensure Meadow House Hospice funds are kept separately and benefit Meadow House patients and staff. We share a charity number (1083634) and governance.

How to refer
Any healthcare professional can refer to the specialist palliative care team, but only with the consent of the patient and/or carer. The patients GP should always be made aware of the referral.
Please download this London Specialist Palliative Care Referral form to make your referral or a referral form can be sent to you from our referrals office.
Call 020 8967 5758 or email referralsmeadowhouse@nhs.net. The completed referral form must be returned to Meadow House Hospice at referralsmeadowhouse@nhs.net
All referrals will be reviewed routinely within working hours in preparation for discussion at the referrals meeting the following morning. These meetings take place Monday to Friday. If the triage team have any questions, we will contact the referrer for clarification.
