The service has a dedicated Caldicott Guardian and several ‘Information Guardians’ as per the Trust’s Information Governance Strategy and all staff work to the Trust’s policies on Data Protection, Data Quality, Confidentiality, Information Governance and Records Management policies and the Information Sharing Protocol developed with partners in care.

Neither party will disclose to any third party, except those involved directly in a patient’s clinical management or where required to do so by statute/DH directive, information acquired during the period of the Agreement which concerns the identity, personal details, clinical condition or treatment to any individual, without the individual’s informed consent.
Meadow House Hospice will not sell, rent, or lease your personally identifiable information to others.
