Meadow House Hospice is committed to respecting and protecting your privacy.

Once you choose to provide us with personally identifiable information (any information by which you can be identified), you can be assured that it will only be used to support your relationship with Meadow House Hospice.

The service has a dedicated Caldicott Guardian and several ‘Information Guardians’ as per the Trust’s Information Governance Strategy and all staff work to the Trust’s policies on Data Protection, Data Quality, Confidentiality, Information Governance and Records Management policies and the Information Sharing Protocol developed with partners in care.

What information we collect

On our web site you can make requests and register to receive materials. The types of information collected on these pages are name and contact information.

Meadow House Hospice uses your information to better understand your needs and provide you with information. We may also use your information to contact you or to provide you with information we think would be of particular interest.

Who we share your information with

The service complies with the Data Protection Act 1998 and the Caldicott Report. Neither party will disclose to any third party, except those involved directly in a patient’s clinical management or where required to do so by statute/DH directive, information acquired during the period of the Agreement which concerns the identity, personal details, clinical condition or treatment to any individual, without the individual’s informed consent.

Meadow House Hospice will not sell, rent, or lease your personally identifiable information to others.

Photographs of patients and their relatives are on the website by permission only.

Security of information

Meadow House Hospice is committed to ensuring the security of your information. We have put in place appropriate physical, electronic, and procedures to secure the information we collect on our website.
